The Commonwealth of Virginia School Entrance Health Form is required for admission to the school. The form is valid for two years. Please ensure that you fill out the front page and sign it at the bottom before submitting it to your doctor for completion. All immunizations must be up to date.

If your child has an allergy that requires medication, please submit both a Medication Administration Form and an Allergy Action Plan form (below). Please provide separate Consent Forms for each medication. Any prescribed medication/s must be in their original packaging with the pharmacy label. Over the counter medications such as Benadryl do not need a prescription but do require original packaging. For children under the age of five, the medication form is valid for one year.

If your child has an allergy that does not require medication, please submit only this form. Please note any relevant details about your child’s sensitivity to the allergen. For example, if your child has a peanut allergy, it is important to note whether s/he cannot eat, touch, or smell the allergen.
